Catholic Healthcare West launches 2011 Community Grants Program

Chandler Regional Medical Center, a member of Catholic Healthcare West (CHW), is seeking applicants for the 2011 Community Grants Program, which award grants to non-profit organizations that are working to increase access to quality care for the underserved populations in the communities they serve.

In addition to outreach efforts for chronic diseases and conditions, this year's grant initiative will support CHW-East Valley's community partnership and outreach efforts in the form of spiritual, emotional and/or physical intervention, prevention and/or management measures for mental health, and child water and car safety. Considerations will also be given to programs focusing on high-risk populations such as elderly, disabled and/or mentally ill, as well as programs that decrease hospital readmissions for patients suffering from chronic disease.
