These are the worst East Valley offenders from restaurant health inspections reviewed July 21-27. For more details, or to search for any restaurant in Maricopa County, click here.

 INDIA GATE

Address: 6245 W. Chandler Blvd., Chandler

Major violations: 6

Inspector's comments: Various foods in the front sandwich prep table and in the walk-in not kept at 41 degrees Fahrenheit or below; the cold water line at the front buffet hand sink was turned off; raw chicken held in a plastic bag stored over a roll of ground beef in the walk-in; the can opener blade and knives stored on knife rack were soiled with organic debris; there were rodent droppings behind the water heater; various knives and a plastic lid were in disrepair, with tips and blades broken; the front sandwich prep table was at 56 degrees Fahrenheit and the walk-in at 56 degrees Fahrenheit; the walk-in door thermometer not working properly; the area around the grease containers was soiled with grease and debris; the hand sink in the kitchen was dripping water onto the floor, the RPZ valve at the bar was installed without a hub drain, and water was draining on paper towels behind ice machine; the back-door sweeps are in disrepair and need to be replaced to prevent rodent entrances; caulking around hand sink in kitchen was in disrepair; exposed wood in kitchen needs to be properly sealed; the Maricopa County Attorney's Office and the Maricopa County Environmental Division Department have initiated an enforcement protocol that could result in a class 3 misdemeanor for establishments that continue to have major violations. 

COPPER KETTLE EXPRESS

Address: 1050 S. Terrace Road, Tempe

Major violations: 6

Inspector's comments: The cutting board, bowls and pans were dirty; an employee's drink in the kitchen did not have the required lid and straw; there were live roaches running up and down the kitchen walls; pots were not stored upside down and were holding water from washing; raw meat was stored above other foods in the walk-in refrigerator; the shelf under the table in the kitchen was dirty; wiping cloths were not being kept in sanitizer between uses; about half the food in the walk-in refrigerator was not date-marked, and food in the walk-in that was dated was older then seven days; boxes of food were on the floor in the walk-in refrigerator; Maricopa County policy requires an establishment that fails to correct repeated violations be placed on probationary status. Failure to permanently correct repeated violations while on probation will result in legal action to permanently revoke permit.

 RED MOUNTAIN RANCH COUNTRY CLUB

Address: 6425 E. Teton Circle, Mesa

Major violations: 6

Inspector's comments: Worker changed gloves without washing hands first; raw beef patties found in holding pan containing lunch meats; four open half-and-half bottles not dated; opened anchovy can in walk-in cooler with a prep date of Oct. 20, can had slight mold growth on exterior; residual food deposits on meat slicer after earlier cleaning; menu consumer advisory does not refer customers to the reminder statement for hamburgers "cooked to order"; worker was smoking at the rear employee entrance even though smoking is prohibited within 20 feet of any opening into building. 

SAN MARCOS RESORT CONFERENCE CENTER

Address: 1 San Marcos Place, Chandler

Major violations: 5

Inspector's comments: Cook used bare hands to prepare sandwich; can opener has dried food debris; soda nozzles had bacterial slime; opened package of luncheon meat was not date-marked; an opened package of luncheon meat marked July 1 was not properly discarded; due to repeated, severe and/or numerous violations noted on this inspection, legal action is being initiated. Failure to permanently correct violations noted on this inspection may result in probation and/or closure of this establishment.

 THE KEG STEAKHOUSE & BAR

Address: 23 S. McClintock Drive, Tempe

Major violations: 5

Inspector's comments: Cold sour cream not held at 41 degrees Fahrenheit or colder; one food service worker washed hands with soap for eight seconds, another worker washed hands without soap; raw eggs were stored above ready-to-eat foods in the walk-in cooler; cracked food service items, not sanitizable; spray bottle was not labeled and had unknown chemical cleaners; found interstacked pans stored wet; bulk white foods were unlabeled; ashtray found 15 feet from front door; found cigarette butts at front door; no state-mandated "No Smoking" signs at each door; due to repeated, severe and/or numerous violations noted on this inspection, legal action is being initiated. Failure to permanently correct violations noted on this inspection may result in probation and/or closure of this establishment.

 SHANGRI-LA

Address: 2992 N. Alma School Road, Chandler

Major violations: 5

Inspector's comments: Several bus containers of precooked chicken and shrimp not date-marked; several roaches noted throughout facility, also rodent droppings noted in dry storage and under bar; coolers and several food contact surfaces soiled with food debris; ice machine was heavily soiled with black slimy material on interior surface; several areas throughout facility are heavily soiled; there was no chemical test kit available; cold potentially hazardous foods not held at 41 degrees Fahrenheit or colder; cook-line cooler temperature measured at 68.6 degrees Fahrenheit; due to repeated, severe and/or numerous violations noted on this inspection, legal action is being initiated. Failure to permanently correct violations noted on this inspection may result in probation and/or closure of this establishment.
