Address: 1159 E. Main St., Mesa

Major violations: 7

Inspector’s comments: Employee cut raw cabbage with bare hands then touched their nose with hand and returned to cutting cabbage; three employees cut cabbage without gloves; raw chicken in walk-in cooler stored over raw beef, raw chicken in plastic bag inside box of produce, raw eggs stored over produce items in walk-in cooler; knives in clean storage under prep area were soiled; employee filled up plastic tote with shrimp at mop sink instead of the prep sink; flour scoops were soiled with wet substance and some substance that has dried inside flour bin; several food items in the walk-in freezer were stored on the floor, boxes of cabbage were sitting on the floor; buckets of salt and seasoning were open and sitting beside the hot water tank; peanuts and other food items in dry storage were open and uncovered; employee washed pans at three-compartment sink, did not rinse pans, then placed into sanitizer basin; area under flour, starch, sugar bins was soiled with dust and debris buildup; areas under cook line and ventilation hood were heavily soiled with grease buildup and debris; there were live insects in the flour and starch, live roaches by the back door and in the cabinets by the soda machine; there was a can of pest spray in the back storage area; there were two large containers of raw eggs on the cook line not at proper temperature; back door does not seal when closed to prevent infestations; not all employees have a food service worker card.

Inspector’s comments: Cooked rice in the cook line reach-in cooler and cooked shrimp in the walk-in cooler were not at the proper temperature; several employees switched tasks (restocking, bussing, cashiering, serving, and cooking) without stopping to wash hands; several food contact surfaces (bus tubs/lids, soda nozzles, prep cooler cutting board, and racks/interior surface of meat and vegetable walk-in coolers) were heavily soiled also, all cooler/freezer handles in the prep areas were soiled; there was no certified food manager documentation available; rice scoop was stored in standing water temperature measured at 81.2 degrees; wall/tiles are damage across from the walk-in cooler, floor between walk-in cooler and freezer were damaged and piping near dishwasher was duct-taped; wall and floor surfaces were dirty along cook line; live roaches and flies were in the prep area; there was no state-approved “no smoking” sign posted at facility entrance; several coolers/freezer gaskets were damaged; three employee drinks did not have lids and straws in food prep area.

Inspector’s comments: Sausage gravy in cooler was not at the proper temperature; corn in hot well was at 115 degrees, not held at 130 degrees or above; the hand sink in the kitchen needs repairs; there were missing panels on the ice maker, needs to be replaced; interior of ice maker had excessive buildup; kitchen and back of establishment is in serious need of deep cleaning.

Inspector’s comments: Employee was eating a bagel and bacon on a food prep counter; worker cut ready-to-eat vegetables with her bare hands; ceiling tiles were missing from the kitchen area; sanitizing bucket in the kitchen area was not labeled; the ceilings, walls and storage shelves were dirty with food debris and grease in the kitchen area; microwave and cooler gaskets are dirty with food debris and grease; back employee hand sink was dirty with grease and food debris.     


Address: 8380 E. Via De Ventura, Scottsdale

Major violations: 4

Inspector’s comments: Sliced turkey, ham and sausage on the cook line were set out for two hours and not at the proper temperature; employee handled raw chicken with gloves, then put on new gloves without washing between; manager is not able to communicate with Spanish-speaking employees; establishment does not speak with employees regarding food-borne illnesses; there was a soiled plate and spatula in hand wash sink; drain pipes were hanging below flood rim of floor sinks; ice machine in kitchen, three-compartment sink and ice bins at bar; sanitizer bucket was not at the proper concentration; a cardboard box of tubes of ground beef were on the floor in the walk-in cooler.
